Terms and Conditions for Outpatient Mental Health Clinic
Appointment Scheduling and Cancellations
Appointments must be scheduled in advance.
Cancellation or rescheduling requires a minimum of 24 hours’ notice.
Failure to provide adequate notice may result in a cancellation fee.
Repeated no-shows or late cancellations may result in termination of services.
Patient Responsibilities
Patients must provide accurate and complete personal and medical information.
It is the patient’s responsibility to follow the agreed treatment plan.
Patients should communicate any concerns or changes in condition promptly.
Confidentiality and Privacy
All patient information will be kept confidential in accordance with HIPAA regulations.
Information may be disclosed only with patient consent or when required by law.
Payment and Insurance
Payment is due at the time of service unless other arrangements have been made.
Patients are responsible for knowing their insurance benefits and coverage.
Any copayments, deductibles, or non-covered services are the patient’s responsibility.
Insurance claims will be filed as a courtesy; unpaid balances remain the responsibility of the patient.
Treatment Limitations
The clinic provides outpatient mental health services and does not offer emergency or inpatient care.
In cases of psychiatric emergency, patients should seek immediate assistance at an emergency room or crisis center.
Treatment recommendations are based on clinical judgment and available information; results cannot be guaranteed.
Termination of Services
Either party may terminate treatment at any time with appropriate notice.
The clinic reserves the right to terminate services if the treatment plan is not followed, payment obligations are unmet, or behavior is disruptive or unsafe.
Telehealth Services
Telehealth may be offered as part of treatment with consent.
Patients must ensure a private, secure environment during telehealth sessions.
Telehealth is subject to the same terms and conditions as in-person services.
